What if one has been requested? Is it possible that they may send one?

Unlikely. I consider myself an average user but visit over 100 sites each day

If they want to keep all that data about their customers, they sure can. The name of every website you go to passes through their servers, so they could log it and report it later. I doubt they’d report it unless asked (and possibly for a fee), but it’s conceivable.
Proxy usage might hide from them the site names — but not the names of the proxies themselves.
